    The Reporting Problem

    Operational quality and recall management functions typically have access to a large set of indicators. Number of complaints opened. Average time to triage. Open CAPAs by age. Field actions in flight. Audit findings by severity. Supplier non-conformance counts. Regulatory submission backlog. Cost of poor quality. Each of these has a place in operational management.

    The board, the audit committee, and — in regulated industries — the senior compliance committee, do not have time for the operational dashboard. They have time for, at most, a small set of metrics that tell them whether the recall and quality programme is actually performing against the obligations the company has accepted, and whether the trend is going in the right direction.

    The reporting problem is therefore not about producing more numbers. It is about choosing the small set of numbers that genuinely inform governance decisions and presenting them with the supporting context that makes them actionable.

    1. Are we currently safe? What is the state of the product population in the field, and what is our exposure to active or imminent safety events?
    2. Are we operationally ready? If a serious event materialises today, can we respond inside our regulatory windows, with a defensible decision trail, in every market we operate in?
    3. Is the trend going in the right direction? Across multiple quarters, are the leading indicators of quality and safety improving, deteriorating, or unchanged?

    The KPI framework we recommend maps directly onto these three questions: a small number of safety-state metrics, a small number of operational-readiness metrics, and a small number of trend metrics.

    The Five-Plus-Two Framework

    The framework we have seen work best in practice consists of five primary KPIs reported every board cycle, with two additional KPIs rotated in based on the period's most significant events. The primary five are:

    KPI 1: Active Field Actions in Flight (Safety State)

    The count of recalls, advisory notices, software updates, and other field actions currently active in any market the company operates in. This is the most basic safety-state metric: at this moment, what is the company asking the field to do about its products?

    The reporting context that matters: action by category, by jurisdiction, by classification severity, and the days-since-initiation distribution. A board does not need to see every line item, but it does need to see whether the field action profile has changed materially since the last cycle.

    KPI 2: Time-to-Notification Performance (Operational Readiness)

    For every notifiable event in the period, the elapsed time from internal decision-to-notify to formal notification of the relevant authority — measured against the applicable regulatory window for that event. This is the most concrete measure of whether the company can perform inside the timelines its obligations require.

    The reporting context that matters: percentage of events meeting the applicable window, the distribution of elapsed times, and the underlying drivers of any breaches. A pattern of "just-in-time" notifications is itself a signal of operational fragility, even if no formal breach has occurred.

    KPI 3: Open Critical CAPAs by Age (Operational Readiness and Trend)

    The count of open critical-severity Corrective and Preventive Actions, with the distribution by age. CAPAs are the operational closure mechanism for quality and safety findings; an aging population of unresolved critical CAPAs is a leading indicator of recurring issues and a point of regulator scrutiny.

    The reporting context that matters: aging buckets (under 30 days, 30-60, 60-90, over 90), the principal sources of the CAPAs (audit, complaint, internal finding, regulator inquiry), and the resourcing position of the responsible functions.

    KPI 4: Complaint Trend with Severity Weighting (Trend)

    A normalised complaint volume metric, weighted by complaint severity, trended over at least the trailing four quarters. Complaint volume on its own is a noisy indicator — it is influenced by sales volume, channel mix, awareness, and seasonality — but a severity-weighted complaint trend is a meaningful leading indicator of safety performance.

    The reporting context that matters: the normalisation methodology (per million units shipped, per active customer, per service hour, depending on the business), the severity classification scheme, and the categorical breakdown that explains the trend.

    KPI 5: Regulator Engagement Posture (Operational Readiness and Trend)

    A composite assessment of the company's current engagement posture with each major regulator: open inspection findings, response status, escalation activity, and notable inspectorate communications. This is the most qualitative of the five metrics, and it is the one that most directly answers the audit committee's underlying question: how do regulators currently perceive the quality programme?

    The reporting context that matters: open findings by regulator and severity, days-since-last-inspection by regulator, and a brief narrative on the most material regulator interactions in the period.

    The Two Rotating KPIs

    The remaining two KPI slots are rotated in each cycle to address the most material risk or opportunity in the period. Examples include:

    • Supplier non-conformance trend in periods following a material supplier change or in categories with concentrated supplier risk.
    • Field-action effectiveness check pass rate in periods following multiple closures, as a measure of recall execution quality.
    • EU Safety Gate or comparable competitor-event exposure in periods following sector-wide regulatory activity that may signal category-level risk.
    • System-of-record adoption metric in periods involving a major platform implementation, where the value of the investment depends on user adoption.

    The Operational Signals Beneath Each KPI

    Each of the primary five KPIs is supported by a small set of operational signals that the quality function should be tracking continuously and bringing to the board only when they materially move the headline metric. The discipline is to keep the operational signals out of the headline view but to be ready to surface them when the conversation requires it.

    For example, time-to-notification performance is supported by signals including the percentage of triage decisions completed inside the internal target window, the on-call escalation completion rate, and the volume of after-hours decision-making.     A KPI framework only becomes a real management tool when the reporting cadence is structured around it. The pattern that works best in practice has three layers:

    • Operational layer — daily review of the operational signals by the quality and recall response leadership, with weekly aggregate reporting to the broader function.
    • Management layer — monthly review of the primary five KPIs by the senior quality and operations leadership, with documented action on any KPI that has moved adversely beyond the established threshold.
    • Governance layer — quarterly reporting of the primary five KPIs to the relevant board or board committee, with the two rotating KPIs selected based on the period's most material developments.

    The management layer is where most underperforming reporting frameworks fail. A KPI set that is reviewed quarterly at the board level but never substantively at the monthly management level becomes a presentation artefact rather than a management tool. The discipline of reviewing the same set of metrics monthly — and acting on movement — is what makes the quarterly board view a genuine reflection of programme state rather than a quarterly construction exercise.
